Family Healthcare, Making Lives Better in Southwest Utah, formerly Southwest Utah Community Health Center, is a private non-profit agency providing primary health care services to approximately 13,000 people every year.

Second clinic location is 2410 E. Riverside Dr. behind Millcreek High School. See the same doctor each time you visit. Same day/next day appointments. Accepts private insurance, Medicaid, CHIP, Medicare and provide sliding fee scale for uninsured individuals.

Prenatal care, delivery, well child checks, women's health care, men's health care, geriatric care, counseling, limited dental for the uninsured. Reduced price prescriptions. We are your family health care provider.

We have Certified Application Counselors who can help you understand your health insurance options, help you fill out an application online or on paper, help you determine which insurance plan will meet your needs, explain the subsidies available for help paying your premiums and more!
